How to get a Library Card

Anyone is welcome to use the materials in the library. A library card is needed to check out materials from the library.

FEES: A resident or owner of property in Wellsville may have a library card free of charge. Nonresidents may check out materials by purchasing a library card. The fee is $7.50 for an individual or $10.00 for a family card per year.

To receive a free card or purchase a card the following must be supplied:

Adults 18 years and older:

  •  Name
  •  Address: proof of residency
  •  Phone or message phone number
  •  Mo. Driver’s License #
  •  Date of birth
  •  Picture identification
  •  Printed and written signature

Parents are responsible for library materials checked out by their children. Parents or guardians must also fill out a "Wellsville Public Library Informed Consent of Use of Library for Minors" form. (a minor by state guidelines is 17 years and younger) Children must be 7 years of age to receive a card.

Renewing cards: All library cards are valid for one year from the date of purchase.

Replacing cards: Lost library cards will be replaced for $1.00.
